Caller Interview Sheet
 This is an old caller interview sheet I used for years.  It was originally designed to be used with birthday party customers but I used it with schools and libraries and just put the school name or the library name where it says "Child".  These sheets are a great way to keep track of important information for your shows as well as for your business (i.e. tracking where shows come from).
